or more correctly:

=COUNTIFS(Sheet2!A:A,lcase(trim(("yes")),Sheet2!C:C,Sheet1!$A1)+COUNTIFS(Sheet2!B:B,lcase(trim("yes")),Sheet2!C:C,Sheet1!$A1)

the trims and the lcases will help with bad data entry, which might not be necessary if you're not relying on people to follow simple instructions
