mistakes...

contracts always not quite as they could be(stupid release clauses, or stupid lock in clauses
keeping managers too long even when their mistakes are transparent
allowing said managers to waste money during those time periods
usual waste of money on non-football related matters (Refer to my husband)
buying unfit players - not occasionally but regularly
buying out players contracts - again not occasionally but regularly
failing to bring in some footballing presence to the board
continuing the pretence of being a fan-friendly club whilst ignoring some of their main wishes
letting real talents of management like Cullen go
failing to monitor the academy's progress whilst still soaking up cash and again putting in remedial steps way too late

That's just off the top of my head
