Unseen Incidnets - Rules etc.

The FA Rules explain how they act and the scope of the commission (p241 of FA Disciplinary Regulations).

I can't find anything about who can raise or report something but it MUST not be seen by officials (Tyrone Mings stamping on Oliveira was similar but Referee saw it so no action could be taken retrospectively)

Still searching for a record of other similar retrospective sanctions applied.
