You have to have team mates on the move looking for a bit of space so you can pass to them. If you pass to them when they have an opponent (especially a bigger, stronger one) standing right by them the odds are they'll lose it. When our defenders/Gilmour get the ball we seem to have our whole team concentrated in a 20-30 yard strip of pitch with nobody in remotely enough space to pass to - hence the number of sideways passes at the back to the only available players.
