Varies a fair bit from place to place.
Here in Cardiff the homelessness/mental health support problems have clearly gotten worse over the last four or five years.
I'd say the drunken loutishness has generally become less common.
People not used to cities can find Cardiff quite intimidating, but I'd still say at core it's a really lovely and friendly place.
When I'm up in Norwich I've seen the same kind of stuff. There's patches of the city which do seem rougher and less safe (pretty much true of everywhere).
Norwich is a million miles better than my last visit up to the Midlands, while never great, I was appalled at the decline of some of the towns. Horrible.
Very patchwork country and our cities are in the process of reinventing themselves. Some more successful than others.
