for smaller firms it's a distinction without a difference. For larger firms an MD will usually report in to a CEO.
CEO implies a broader range of responsibilities whereas MD implies focus on running a particular division or country/region of operation, within parameters set down by the CEO and board.
The CEO sometimes sits on the board and sometimes doesn't but always reports into the board and will normally attend all board meetings. The Chairman is responsible for his/her recruitment, appraisal and dismilssal; in larger organisations the SID (senior independent director) also plays a key role there.
For Norwich I think it's more to emphasise that the MD will take charge of all non-football operations whereas the sporting director will take charge of all football-related matters. So the MD is responsible for shoring up the balance sheet, building a fan base (e.g. by lowering prices for under-21s/students), leveraging that fan base for income (through replica shirts, match tickets, highlights packages, targeted base to sell advertising against and so on). The sporting director is accountable for on-the-pitch results and operates within financial parameters set by the Board (after negotiation with them).
