He was constantly blaming basic errors by the players for everything

Now we could all see that this was happening a lot, but it must have been pissing off the players. In particular when his team selection and tactics were hardly perfect, and I don't recall him coming out and saying "I ballsed up my team selection", or "I shouldn't have made that substitution ". It was always the players letting him down. That sort of approach will see loyalty eroded pretty quickly.
