So which of these obvious failings was rectified? (some answers have a delete where applicable option.)

Have we got rid of all our rubbish defenders & replaced them with top quality ones from big clubs / young hungry ones from little clubs?
Have we sacked Irvine and recruited a decent defensive coach?
Have we completely changed our fitness regime?
Have we sacked AN?
Did we replace the useless Murphy/Naismith/Pritchard with the criminally underused Sergi Canos?
Did we replace the useless Murphy/Naismith with the criminally underused Alex Pritchard
Have we stopped making stupid substitutions, like bringing on an extra defender to protect a slim lead?
Did we sniff up the USB?
