All the things you say are true. And they show it was accidental (as opposed to intentional).

Almost all great disasters are born of a similar list of cock ups. Take the Buncefield refinery explosion. Inadequate staff, bad training, poor equipment badly maintained ... An accident

No doubt you'll explode with rage and accuse me of being disingenuous. But it's obviously correct.

The cover up was NOT an accident.
