Reverse happened to me once, as runners-up we couldn't get promoted as the first team were in the division above. Actually, IIRC, we were Div3 RU (just checked trophy cabinet!) and they got relegated from D1 so stopping our progress. They never called on players from our 'reserves' too, so had they've done that they might've stayed up!
