Lambert was probably thinking 'keep Holt on the pitch because he is good at holding up the ball and drawing a foul, so keeping pressure off the defence. Bring Jackson on to try and catch Stoke on the break with his pace'.
The problem was that it left only 3 in midfield so they dropped back too deep, they lost the width, and had to play it long to the forwards.
Under the circumstances, I think PL should have brought on Wes rather than Jackson, and Fox rather than Surman.
Another option could have been to play R.Martin wide in midfield to help stop Stoke crossing the ball.
